That rootstock is done for, but the scion looks like it might be savable. You will have to make the final decision on that. If it’s getting too squishy, the next best thing you can do is save a pup and graft it. You’ll need a new columnar cactus to graft the scion onto, so head to your local garden store. Pretty much any species can do, so find one you like, preferably one on the taller side. If you don’t have any good garden stores in your area with cool looking cacti, you can head to some place like Lowe’s or Home Depot to get some more grafted moon cacti. You can probably find one already with a dead scion, and (at Lowe’s at least) you can get the Distressed Plant discount.
Once you’ve got a healthy specimen, it’s time to graft. All you need for this step is a sharp razor blade, some alcohol for disinfecting, and something like painter’s tape or a nylon sock to hold the two organisms together. I’d recommend watching Youtube videos on this before attempting.
Give your new Frankenstein monster two weeks to fully fuse together. Once that’s done, you can take preventative measures to keep this from happening again by repotting in better soil. Lots of people have formulas they swear by, but the cheapest solution is to take the dirt it came in, and mix it with an equal part of perlite.
