First cactus I’ve been able to keep alive for a couple years, would love opinions on what to do to keep it healthy or to improve?

That plant is not healthy my friend. It’s very very etiolated (searching for light there for growing abnormally long and thin).

    Give it light!

Cool string of pearls in there with it. I was given one that wasn’t doing well at all so I took cuttings. I now have 4 of them growing like mad😁

I yearn for light father… I’m stretching with all my might father

  4. Imagine posting this only to find out you have been starving your plant of light

Thanks guys for the suggestions, I moved it to the other window and if I don’t see improvement soon I’ll put it under a spare aquarium light

I would pull off all the stretched pups a discard. Seriously consider chopping the stretched bits on the base plant. Well formed future pups will fill in and hide scars…

Well I can tell which direction the window is…
