After attempting some image searches the answers I got weren't clear and after consideration I cut off the tomato and washed my hands well. I was wondering what could have caused this? Top image results were disease and potentially hornworms.

Not hornworms. They will eat the entire plant in a day or two, but I’ve never seen them bore into a fruit. this is likely sucking insects like leaf footed bugs. Looks like they hit it when it was smaller and it tried to heal the injury.

Squirrels or birds in thirst maybe? I have a water feeder near mine to keep them at bay

I’ve seen birds go for a tomato seed snack and leave them looking like that in my yard, but usually when the fruit is a little more ripe.
