I think it’s so small because the leaf isn’t from a top cutting but from an activated node. It will rapidly be getting bigger leaves. The hole looks like mechanical damage to me, I also get those fron time to time.
Don’t worry about the size, they size up fast- if you’re lucky the 3rd or 4th leaf can have fenestrations (given sufficient lighting)
