This is pretty badly etiolated, it needs much more light first and foremost.

Crassula perforata (aka string of buttons) or one of the other very similar crassulas. It’s etiolated and needs a gradual acclimation to a lot more light than it’s been getting. Once there’s compact, healthy growth, behead and reroot the compact sections.
I’m also seeing lots of desiccated stems in there. Cut all of those desiccated bits away and reroot. I’d also suggest repotting into a much grittier substrate. You generally want to aim for at least 50% inorganic grit in your substrate.
