Thoughts ?

Planted these fem amnesia lemons seeds 5 days ago and all have germinated except this lil girl. Moved the little piece of rapid rooter I had plugging the hole and found cotyledon leaves have left the shel and look like it may be tangled with the stem possibly..
Also it seems there is small roots growing up near the surface that can be seen around the hole.
Anyone seen this before
