First attempt was about 75% potting soil mixed with some random pond water and maybe a worm or two. However, I feel like I may be on the right track with this one. Any suggestions/ideas/improvements?

Pond water with sand, some rocks, plants, and a large stick that had some plant roots on it. I made sure I grabbed a good bit of plants.

These pics are day 3. The water is starting to get clearer.

    I think you’ve got the right idea. The issue may be if the plants you grabbed are the kind that grow above the water and can get big. If so they’ll take up all the nutrients in your jar and everything will go to crap. The ratio of sediment to water to air looks good though, so if the plants you have are suitable for a jar it has a chance of working out long term.

    Take 5 handfulls of dirt, put it in a bucket, spray hose into it until buckets full and empty bucket,carefull not to spill the heavy stuff, do it 5 times, put a layer in the bottom. take river sand (concreting sand) wash it the same exact way, now add add a layer of sand same thickness as the dirt, so that 1/3 of the jar is full, Add some rocks in a pile and your water plants then add your river/pond water careful to filter out any leafy debris, fill to 2/3rd mark. Enjoy.
