Discovered what looks like insect damage at the base of one of my red oaks. Looking for advice on treatment and confirming that is what is damaging my tree. This was not here last year, I put the paper and mulch down before last winter and didn’t notice this.

Was your tree dead or dying before you put that paper and mulch down?

Too much mulch and that paper is weakening your tree and likely exposing it to insects. Remove the paper completely and mulch back to an appropriate height. Depending where you are red oaks are pretty susceptible to insects and parasitic plants such as mistletoe.
