When I got it about six months ago, a couple of stems grew initially, but it has begun to decline recently. Nothing has changed—I keep it away from direct sunlight and water it once a week. I've started adding feed to the last two waterings in hopes of improvement, but this is now the second stem to die.

Looks overwatered. You should not water on a schedule but only when the first 1-2cm of the soil is dry to the touch.
