Hard to say exactly why but the union didn’t form properly and the scion dried out. Unfortunately, probably too dry to retry. Slab grafts can dry relatively quickly, a puck might be a good place to start, should dry slower and give you more chances if it doesn’t take

I have started using cellophane (food film) to hold the scion on; covering most of the cut surface but open enough to vent and allow exchange of air. It slows down the drying of stock/scion but doesnt allow for excessive condensation.
