At first it had good light, then grew the skinny part because it didn’t have the right amount of light, and now it is getting much better light so it is growing the top part thick again like it was meant to. As u/Bradsohard69 mentioned, the skinny part is the result of etiolation.

Like others have said, etiolation. The cactus started growing the skinny bit when it wasn’t getting enough light, and when the light got better, the growth returned to normal, resulting in the spicy dumbbell you have here.
The skinny bit never quite thickens out even after the cactus starts getting enough light, so it may become a bit top heavy as it gets bigger. You might wanna look into cutting off the top bit and replanting it on its own if the cactus ever starts having trouble supporting itself under its weight.
