What is the best practice method for creating this look? Typically I’ve seen flagstone installed with a stone base and joint filler. If we wanted to use something like mondo grass instead of a joint filler, how can that be done given the stone base?

It depends on the soil type under the stones. Ive done a patio like this with heavy clay soil and they can lay directly on top, with a little sand for leveling. Leave about a three inch gap for the Mondo grass; add some compost as you plant.
