I'd be genuinely surprised to see a testicular torsion caused by blunt force trauma to the suprapubic region. From the video suggesting a blow to the mid-suprapubic region or RLQ, the primary differentiation to be made is between injury to superficial vs deep structures. Deep, intraabdominal or intrapelvic injury is more worrisome, but less likely. Bladder rupture should be ruled out urgently, and appears to have been. None of the videos suggest testicular or inguinal canal injury. Retroperitoneal structures like the ureters and lumbosacral plexus are very unlikely since these structures are deep and fairly well isolated. That leaves the abdominal wall as the most likely site of injury, and that's most consistent with the video, as well as his behavior immediately after the blow. This kind of injury is fairly benign in that no organs are injured, but it can be intensely painful, and will severely limit his ability to run and make quick postural and balance adjustments. And they take weeks to resolve, particularly if a hematoma (a pocket of blood) develops within one of the three abdominal muscle layers. A rectus sheath hematoma is a more dire possibility, but his injury is a bit low for that.
Need an update for the actual injury; pelvic contusion.

Dude I suffered a sports hernia this summer and omg is it awful, pain everywhere, can't do a sit, and pain in my gut whenever I sneeze or cough.

Hey Brian! Athletic Trainer here. Weird thing I've seen in my practice with internal organ injuries I want to share. I had a kid lacerate his spleen during football practice last year. Category IV laceration and he tested negative for Mono🤷♂️. His only symptom was feeling like he got the breath knocked out of him. No significant pain. No other immediate symptoms. Held him out of practice because he couldn't catch his breath. Later he said he felt much better and went to class. Went to the entire school day and went home feeling fine. Woke up that night vomiting was taken to the ER. Had surgery that night. Still the kid swears he never had any pain. Only that winded feeling. Strange one for sure.
I don't understand why they need to remove the spleen or any organ due to injury. Is it as simple to say that they have become irreparable?
