It’s a fused bloom or megabloom. You can either let it go, and it will likely produce a really big frankenmater, or pluck it. On the plus side, those big chonkers can be so interesting (cat-facing is what it’s usually called, when you have like seams and lines in the tomato, often found on heirlooms), but on the negative side, it takes more energy for the plant to produce, sometimes taking energy away from the other fruit coming on. So it’s your call to keep it or no.
