I’ve had this little plant for several years now, but over this winter, it has started to slowly die, I think.
Any idea what’s I’m doing wrong? Too much water? Not enough? Too much sunlight? Not enough? Just too old? Needs to be repotted into a larger container?

Probably watering too often and not enough sun. I would repot in a slightly larger terracotta pot. Use very gritty soil. Give it as much light as possible and water thoroughly only when the pads have a slight wrinkle.
The bottom of the rightmost piece is rotten and will have to be propagated.
