I spoke with a cactus expert about my spiral cactus recently (my cactus looks similar to yours) and he said that the spots were from under watering. He said that spirals are a bit thirstier than some other types of cacti.

Yours looks a little thirsty to me from the ribs but just keep it in mind over time since you don’t want to risk overwatering and hurting the roots

It’s fungal. Use systemic fungicide on it.
