Do you know what cactus this is, it reminds me of a Lophocereus schottii monstrose.
I don’t know if they are capable of producing pups once the top growing point has been removed, however the section which I have indicated as needing to be cut and removed won’t support the weight of any heavy growth.
Therefore I would advise cutting and removing the section between the red lines. The section below the bottom red line already has roots and might pup for you. The section above the top red line can be left to harden over/callous and can then be rooted in a gritty growing medium.
The top growth looks really good, but obviously there is an etiolated area in the middle. Personally I would let the top bit grow a little more and then plan to cut away the skinny part entirely. Then you’ll have the base create a pup from a strong section and you’ll have the new healthy-sized cutting from the top to replant.
We are getting close to fall and cold temps, not sure where you’re located but that would give us a better idea of how to save him.

If there’s a worry that the bottom wouldn’t grow pups, could you cut out the middle and prop the top back on the lower base that’s leftover? Might keep the overall feel of the original cactus.
