Hello!
Currently my plant leaves that are yellowing and dying as well as roots. I have it in a low light setting about 12 feet from a window and it has only been in water for the last year (approx.). I added the fertilizer shown in the last photo (only about 5 drops) but I’m unsure if that is okay.
I don’t know what kind of plant this is as I was gifted it by a neighbor who moved states.
If I could get an identification and what is wrong with it, along with care tips that would be great.

The Roots look great. I think you’re just having some natural die off.
Plants kept in water can be a bit paradoxical, my best guess is that they are not getting enough light.
Those roots are ready to go into soil! There aren’t a whole lot of nutrients in water so getting it into soil will make it happy and stop the yellowing of the leaves. The leaves won’t turn back green, but you’ll get new growth in no time.
If you want to keep it in water, there’s a lot to learn about PH, EC, fertilizer (but not too much), etc. I use water for propagation but I never keep my plants in water for more than 4 months.
I believe it’s a Dieffenbachia. It probably wants to be closer to the window than 12 feet. The best pot would be 3 inches or 4 inches diameter, or a clear version of a red solo cup filled only halfway with soil.
