That’s a normal growth pattern for H. maughanii and other Haworthia species and hybrids. What’s unusual about the pup? Explain what doesn’t make sense?

I think that’s just another pup tbh.
Maughanii do offset, some plants more than others. I have a distinct clump forming one that’s about 6 heads.
Bifurcation is a division of the growing point usually resulting in 2 fairly equally sized heads forming where the old singular head was. These heads tend to drift apart as they gain in size.
Assuming you mean the small growth at about 3 o’clock, that’s just another pup forming where they often do – just within the outer ring of leaves.
