Hey, so this survey is pretty confusing and you will probably get bad data.
You’re asking people to rank from 1st to 4th place in the instructions, but you are then giving star ratings, where more stars is usually better. That’s going to muddle any results you get. Also, people are definitely going to miss the instruction to not put the same rating on different items.
What you want for this in Google Forms is:
* a multiple choice grid question type
* anchored column choices – my suggestion would be “1st (Favorite)”, “2nd”, “3rd”, “4th (Least favorite)”
* set to require a response in each row
* set to limit one response per column (for forced ranking)
* set to shuffle row order
There are options for the latter two under the lower right three-dot menu in the form builder.

What /u/hyouko said about the issues you’re going to have from the ranking scheme on the form is absolutely correct.
